Liability Waiver
I, the undersigned, understand and acknowledge that play on an amusement device
entails both known and unknown risks, including, but not limited to, physical
injury, form falling, slipping, crashing and colliding. Emotional injury,
paralysis, distress, damage or death to any participant. I hereby voluntarily
and expressly release, indemnify, forever discharge and hold harmless Extreme
Rentals LLC from any and all liability, claims, demands, causes or rights of
action, whether personal to me or to a third party, which are in any way
connected with participation in this activity, including those allegedly
attributable to negligent act or omissions. Should Extreme Rentals be required
to incur attorney's fees and cost to enforce this agreement, I expressly agree
to indemnify and hold Extreme Rentals LLC harmless for all such fees and cost.
In the event, I the undersigned or any of my participants file a lawsuit against
Extreme Rentals LLC, it is agreed to do so solely in the state of Georgia. I
agree that if any portion of this agreement is found to be void or unenforceable
the remaining portions shall remain in full force and effect. In consideration
of being permitted by Extreme Rentals LLC to use its equipment and facilities,
the undersigned and/or their participants agree to indemnify and hold harmless
Extreme Rentals LLC. From any and all claims which are brought by the
undersigned and/or their participants and which are in any way connected with
such use or participation. A set of Rules and Directions are either displayed on
the bounce units or have been provided to the undersigned which I agree to
follow and utilize at all times and during operations and use of the unit(s).I,
the undersigned, acknowledge and certify that I have had a sufficient
opportunity to read and acknowledge the risks. I understand it's content and
that I execute it freely and without duress of any kind and agree to the terms
herein stated.

Operator Safety Acknowledgment Form

_____ I understand that I am responsible for the equipment until the rental company arrives to pick it up.
_____ I understand that while the children are on the inflatable equipment, a responsible adult must be supervising.
_____ I will ensure that all riders remove their shoes and any sharp objects before entering the inflatable. Sharp objects include eyeglasses, toys, jewelry, and any item that could scratch the unit or another rider.
_____ I understand that no food and drinks are not allowed inside the inflatable, including gum.
_____ I understand that no kind of spray sting is to be used near the inflatable because it can severely damage the unit.
_____ Unless the unit is specifically designed to be a water ride, the inflatable should not get wet.
_____ I understand that no kind of soap or shampoos are to be used on any of the inflatable units, they make the unit too slippery and can cause injury.
_____ I will not allow riders to do flips, climb the netting or roof areas, or roughhouse in the inflatable. I will remove riders who do not follow directions to ensure the safety of all riders.
_____ I will not allow riders to sit on the safety step.
_____ I understand that riders should be sorted by size to prevent injuries. Only children of approximately the same size should be in the inflatable at any time.
_____ I agree not to exceed the recommended number of riders or weight limit. See Safety, Operation and Emergency Procedures Agreement.
_____ Safety and evaluation procedures in case of unit deflamation have been review with me.
_____ I am aware of how the weather can effect the inflatable and I have reviewed the procedures and understand what to do in the event of bad weather (rain or wind).
_____ I understand that riders must slide in a sitting position. Feet first, Jumping and flipping from the top of the slide is not permitted and can cause injuries.
